Completed forms can be:

• uploaded as a secure message in the Athena Patient Portal. (You do not need to be an existing patient to use the Portal; simply click the Patient Portal button on our home page to create an account.)
• emailed to our office at mlipp@chambersandblohm.com. Please note that, if you choose to email, you accept the inherent risks of email communication, including that we cannot ensure confidentiality.

Note that the “Psychological Testing Payment Agreement” is only required for those with a psychological testing appointment.
